Cayman Islands passports are issued only to British Subjects who are citizens of the United Kingdom and Colonies. but may in certain circumstances be issued to British Subjects without Citizenship and to British Protected Persons. Citizens of other Commonwealth countries should apply for passport facilities to the nearest representative of that Commonwealth country. HOW TO COMPLETE FORM 1. The form should be clearly written in ink using block capitals for all names. Sections 1, 4, 6 and 7 of this form must be completed on behalf of all children. Sections 2, 3, 5 and 8 must also be completed where applicable. The form should be completed and signed by one of the child’s parents. If the person signing Section 6 is not the legal guardian (normally the father or widowed mother) there will be some delay in the issue of the passport until a check has been made with the Passport Office that the legal guardian has not lodged an objection to the grant of passport facilities to the child. 2. Authentication of Application. The application must be countersigned in Section 7 by a Member of the Legislative Assembly, Judge of the Grand Court, Magistrate, Justice of the Peace, Minister of Religion, Clerk of Courts, Police Officer (Gazetted), Bank Manager, Medical Practitioner, Attorney-at-law, who is a British subject ordinarily resident in the Cayman Islands and who has been personally acquainted with the applicant. 3. Photographs. Two copies of a recent photograph (not coloured) of the child must be included with the application. These must be taken full face without hat, and must not be mounted. The size must not be more than 2 1/2 inches by 2 inches or less than 2 inches by l 1/2 inches. They must be printed on normal thin photographic paper and unglazed on the reverse side. The person who countersigns the application is also required to endorse the reverse side of one copy with the words: “I certify that this is a true likeness of....................................” and add his signature. 4. Documents to be produced. 10 Passport Regulations (1998 Revision) (a) If the child was born in the United Kingdom and Colonies— child’s birth certificate or child’s previous passport (see footnote). (b) If the child was born outside the United Kingdom and Colonies of a British father- (i) child’s birth certificate; (ii) father’s birth, naturalisation or registration document, or other evidence of British nationality (see footnote). (c) If the child is a citizen of the United Kingdom and Colonies by naturalisation, or by registration-naturalisation or registration document (see footnote). (d) A change of name, except in cases of adoption when the original name is not required, must be substantiated by the production of evidence showing that a bona fide change has been made. (e) If other children under 16 years of age are to be included in the passport (see Section 5 on page 3 of the form) their birth certificate must be produced. Only children who are citizens of the United Kingdom and Colonies or British Protected Persons may be added to a British Passport. Footnote: Documents are not normally required if a passport showing the child’s national status as a British Subject, Citizen of the United Kingdom and Colonies, is being surrendered. SUPPLEMENTARY INFORMATION FOR OFFICIAL USE ONLY Amount of fee paid Notes for guidance of applicants NOTES FOR GUIDANCE Cayman Islands Passport Application Form l Since the coming into force of the British Nationality Act 1948, on 1st January, 1949, Cayman Islands passports are issued primarily to British subjects who are also citizens of the United Kingdom and Colonies, but may in certain circumstances be issued to British subjects without citizenship and to British Protected Persons. Citizens of other Commonwealth countries should apply for passport facilities to the Office of the appropriate High Commissioner. HOW TO COMPLETE THE FORM TO AVOID DELAY THE APPLICANT SHOULD COMPLETE ALL RELEVANT SECTIONS OF THE FORM CLEARLY IN INK 1. SECTION 1 should be completed in block capitals by all applicants. If possible please give a telephone number: it may be necessary to communicate with you urgently. 2. SECTION 2 should be completed by all applicants. Your passport will be made valid for all countries in the world. It is, however, to your advantage to state the countries you propose to visit and the purpose of your 15 Passport Regulations (1998 Revision) journey; by doing so you will enable us to advise you about passport problems which may arise in certain countries. 3. SECTION 3 should be completed In block capitals if particulars of your wife are to be included in your passport. If your wife has been married more than once details of any previous marriage(s) should be stated in Section 13. If your wife is a British subject or citizen of the United Kingdom and Colonies by descent details should be stated in Section 13. (see also Note 6). If, on or after 1st January, 1949, you married a woman who was not a British subject at the time of the marriage her particulars cannot be included in a family passport unless she has since become a British subject by registration. Nor can she be included if she is a citizen of another Commonwealth country and does not also possess citizenship of the United Kingdom and Colonies. A wife cannot use a family passport to travel without her husband (see also Note 9). 4. SECTION 4 should be completed in block capitals if children under 16 years of age who are related to you are to be included in the passport (see also Section 10). Children cannot use the passport to travel without the person to whom it was issued. Young persons aged 16 years and over need separate passports. Application for a separate passport for a child under 16 at the time of application should be made on form 2. Only children who are citizens of the United Kingdom and Colonies or British protected persons may be added to a British passport. 5. SECTION 5 should be completed if you are a British subject or citizen of the United Kingdom and Colonies by naturalisation or registration. If your wife and/or children were naturalised or registered, and their particulars are to be included in your passport, please complete the second and third columns as appropriate. 6. SECTION 6 should be completed if you were born outside the Cayman Islands. If you were born in a foreign country, in the Irish Republic, or in a part of the Commonwealth other than a place which is still a colony, you may be able to claim citizenship of the United Kingdom and Colonies through your father if he was born, naturalised or registered in the United Kingdom or a place which is still a colony. If, however, your father also was born outside the United Kingdom or a 16 Passport Regulations (1998 Revision) place which is still a colony, you should state in Section 13 the place, country and date of birth, or claim to British nationality, of your paternal grandfather. 7. SECTION 7 should be completed by a married, widowed or divorced woman applying for a separate passport. Part (A). If you were born in the Cayman Islands and you are surrendering a previous Cayman Islands passport in the same married name you need not complete this part. If you have been married more than once, details of any previous marriage(s) should be stated in Section 13. Part (B). You need complete this part only if you were born outside the Cayman Islands. If your husband and also his father were born outside the Cayman Islands or a place which is still a colony, the place, country and date of birth or claim to British nationality of your husband’s paternal grandfather should be stated in Section 13. 8. SECTION 8 should be completed and signed by all applicants. If you have held a United Kingdom passport, a Commonwealth passport, a foreign passport or other travel document of any description you should cross out A and complete B and submit your previous passport or travel document with this application. If you are included in your husband’s British passport, you should submit it with this application so that your particulars may be deleted from it. If you have held a passport or travel document which is lost or not available, you should cross out A and B, sign the Declaration and complete Section 12. 9. SECTION 9 should be completed and signed by your wife if her particulars are to be included in your passport (see also Note 3). Any previous passport or travel document held by your wife should be submitted with this application. 10. SECTION 10 should be completed and signed by either parent (or an adult acting in loco parentis) if you are aged 16 to 18 and are unmarried (unless you are serving in Her Majesty’s Forces) or if a child under 16 is to be included in the passport. If the person signing Section 10 is not the legal guardian (normally the father or widowed mother) and consent to the inclusion of children or the issue of 17 Passport Regulations (1998 Revision) a passport to a person under 18 has not been given in a covering letter signed by the legal guardian, there will be some delay in the issue of the passport until a check has been made with the Passport Office that the legal guardian has not lodged an objection to the grant of passport facilities to the child. 11. SECTION 11 When you have completed the form, Section 11 should be countersigned by a British subject ordinarily resident in the Cayman Islands who has known you personally for at least two years and who is a member of the Legislative Assembly, Judge of the Grand Court, Magistrate, Justice of the Peace Minister of Religion, Clerk of Courts, Police Officer (Gazetted), Bank Manager, Medical Practitioner, or Attorney at Law. A member of your family should not countersign. (see also “Photographs” below). 12. SECTION 12 should be completed and signed if your passport is lost or not available for current use. The Declaration at Section 8 should also be signed. A new passport can be issued only after exhaustive enquiries. PHOTOGRAPHS Please submit two identical black and white copies of a recent photograph of yourself and also two of your wife if you are applying for a family passport. You need not submit photographs of children to be included in your passport. The photographs a should be taken full face, without a hat. The size should not be more than 2 1/2 inches by 2 inches (63 mm. by 50 mm.) or less than 2 inches by 1 1/2 inches (50 mm. by 38 mm.). They should be printed on normal thin photographic paper, be unglazed on the back and unmounted. The person who countersigns your application (see Note 11) should also endorse the reverse side of one photograph with the words “I certify that this is a true likeness of the applicant Mr. (Mrs. or Miss) ____________________________________” and add his signature. If you are applying for a family passport one of your wife’s photographs should be similarly endorsed by the same countersignatory. DOCUMENTS TO BE PRODUCED The following tables show which supporting documents you should produce with your application. Please read all four tables carefully and make sure that you submit the relevant documents. Photostat copies of birth, marriage, naturalisation, or registration certificates are not acceptable for passport purposes unless officially certified. 18 Passport Regulations (1998 Revision) In some cases you may be asked for additional information to establish national status in the light of legislation relating to Commonwealth countries. (d) If you were a British subject before 1st January, 1949 through your own or your father’s birth or naturalisation in British India or in the Irish Republic and have remained a British subject (without citizenship). 19 Passport Regulations (1998 Revision) All applicants- Documentary evidence of your birth or naturalisation in British India or in the Irish Republic or documentary evidence of legitimate descent from a father born or naturalised in those territories. (e) If you were born outside the United Kingdom or a place which is still a colony, and acquired citizenship of the United Kingdom and Colonies by adoption on or after 1st January, 1950. All applicants- Birth certificate (or previous passport). Marriage certificate. Documents establishing your husband’s (or former husband’s) nationality as at (a), (b), (c) or (d) above. TABLE 4 If name has been changed otherwise than by marriage or adoption. Documentary evidence (e.g. deed poll, statutory declaration) that your name has been changed for all purposes. Similar evidence (if appropriate) in respect of your wife or children if they are to be included in your passport. Footnotes (i) Documents are not normally required if you are surrendering a passport (not a British Visitor’s passport) showing your national status as British subject, citizen of the United Kingdom and Colonies, British subject or British protected person. Documents are requested if you are surrendering only a British Visitor’s passport.
