CONSENT FORM OF RECIPIENT Overview on consultation for transplantation A consultation for transplantation involves having tests, procedures, imaging and consultations with other doctors. It also includes meeting with many members of the transplant team. All these consultations, tests and procedures will help us to know if transplantation is the right treatment for you. It will also help us to know if you are well enough to have transplant surgery. The tests will also help us to know if there is any other treatment besides a transplantation that can help you. In these consultations you will meet a transplant nurse coordinator, transplant surgeon, social worker and other doctors as required. There will also be discussion on other aspects of transplantation, including Rwandan law on donation and transplantation, transplantation procedure, potential surgical, financial and psychosocial risks and alternative treatment options. Upon completion of the necessary tests and consultations, there will be a formal discussion of your results at the meeting of the Committee in charge of verifying persons fulfilling requirements for a donor or recipient. At this meeting it will be determined whether or not you are eligible for transplantation at our transplant center. Transplantation risks: (a) surgical risks: surgical risks include primary graft non function, delayed graft function, artery or vein thrombosis, urinary complications such as leaks and strictures, bleeding, clots, etc.; (b) medications after transplantation: after undergoing transplantation, you will be required to take a number of immunity suppressing medications designed to prevent the body from rejecting your new organ, tissue, cells or products of the human body on a lifelong basis. These medications are called immunosuppressant or anti-rejection medications. You will also require medications to prevent infections or potential side effects that may develop as a result of transplantation; (c) opportunistic infections: the type of infections that we are most concerned about after your transplantation are called opportunistic infections. These may be caused by viruses, fungi or bacteria and may affect people who do not have a “normal” immune system. The organisms that cause opportunistic infections can be present in the environment or may be in our body in controlled numbers. FORMULAIRE DE CONSENTEMENT DU RECEVEUR Aperçu sur la consultation pour la transplantation Une consultation pour une transplantation implique des tests, des procédures, une imagerie et des consultations d’autres médecins. Cela comprend également des rencontres avec de nombreux membres de l’équipe de transplantation. Toutes ces consultations, tests et procédures nous aideront à savoir si la transplantation est le bon traitement pour vous. Cela nous aidera également à savoir si vous êtes assez bien pour subir une transplantation. Les tests nous aideront également à savoir s’il existe un autre traitement que la transplantation qui pourrait vous aider. Au cours de ces consultations, vous rencontrerez une infirmière coordinatrice en transplantation, un chirurgien de transplantation, un travailleur social et d’autres médecins selon les besoins et il y aura des discussions sur d’autres aspects de la transplantation, y compris la loi du Rwanda sur le don et la transplantation, la procédure de transplantation, les risques chirurgicaux, financiers et psychosociaux potentiels et les options de traitement alternatif. Une fois les tests et les consultations nécessaires terminés, il y aura une discussion formelle de vos résultats lors de la réunion du comité chargé de vérifier les personnes répondant aux exigences du donneur ou du receveur. Lors de cette réunion, il sera déterminé si vous êtes admissible ou non à une transplantation dans notre centre de transplantation. Risques de transplantation: (a) risques chirurgicaux : les risques chirurgicaux comprennent la non-fonction primaire du greffon, la fonction retardée du greffon, la thrombose artérielle ou veineuse, les complications urinaires telles que les fuites et les rétrécissements, les saignements, les caillots, etc.; (b) médicaments après la transplantation : après avoir subi une transplantation, vous devrez prendre à vie un certain nombre de médicaments immunosuppresseurs conçus pour empêcher le corps de rejeter votre nouvel organe, tissu, cellules ou produits du corps humain. Ces médicaments sont appelés immunosuppresseurs ou médicaments antirejet. Vous aurez également besoin de médicaments pour prévenir les infections ou les effets secondaires potentiels pouvant survenir suite à la transplantation ; (c) infections opportunistes : le type d’infections qui nous préoccupe le plus après votre transplantation est appelé infections opportunistes. Ceux-ci peuvent être causés par les virus, les champignons ou les bactéries et peuvent affecter les personnes qui n’ont pas un système immunitaire « normal ». Les organismes qui causent des infections opportunistes peuvent être présents dans 57 Official Gazette n° Special of 12/07/2024 l’environnement ou dans notre corps en nombre contrôlé. Chez les personnes qui ne sont pas immunodéprimées, le nombre de ces organismes est contrôlé par le système immunitaire naturel du corps ; (d) risques psychosociaux : je comprends qu’il existe des risques psychosociaux potentiels qui peuvent inclure la dépression, le trouble de stress post-traumatique, l’anxiété généralisée et les sentiments de culpabilité ; (e) risques financiers : je comprends qu’il y a des risques financiers potentiels liés à la transplantation.
