On the conviction of a person of an offense in this chapter, the court or, if directed by the court, the clerk of the court shall provide the convicted person's name, case number, date of conviction and crime convicted of and, if known, the convicted person's social security number, date of birth, address and license or registration number to all of the following: 1.
